Family Support Request

If your family would like to request assistance from the Megan Weisenbach Foundation, please complete the online Family Support Request application. Once the form has been submitted, the Foundation will receive your request by email.

All requests are reviewed by the Foundation’s Family Support Committee, and you will be contacted within 30 days regarding the status of your application. Please be sure to complete all contact information so we can reach you. Providing as many details as possible about your child’s needs will help us better understand how we may be able to assist your family.

Because we strive to help as many families as possible each year, the Foundation is unable to fully fund large items such as handicap-accessible vans, wheelchairs, or similar equipment. 

However, we may be able to provide partial assistance toward these items, so please include this information in your request.

Please note that the Foundation does not provide financial assistance for:

• Mortgage or rent payments
• Car payments
• Insurance payments
• Utility bills

Due to the increasing number of Family Support Applications we receive, we kindly ask that families do not request more than the maximum assistance amount allowed. Our goal is to support as many families as possible each year.

To process your request, the Foundation requires families to upload a letter of support or medical documentation from your child’s doctor confirming the child’s primary diagnosis and, if applicable, any secondary diagnoses.
